Through Balance Health Plan, Champion delivers a structured, team-based care model that helps members

The program combines proactive care coordination, caregiver education and support, and ongoing clinical oversight to address both the medical and non-medical needs of individuals with dementia. Balance Health Plan extends Champion’s commitment to high-touch, community-based care to one of Medicare’s most complex and high-impact populations, aligning clinical outcomes, caregiver support, and total cost-of-care reduction through a CMS-sponsored innovation model.
